Health  Assessments:  Physicals for entering ninth graders are due by October 15, 2015. Physicals must be on the CT Health Assessment form commonly known as a “blue form”. The physical has to have been done by August 31, 2014. This is the only physical required for high school unless you child is participating in athletics.

Sports Physicals:  Sports physicals are required yearly for students participating in sports.  Sports physicals are valid for one year and must be submitted to the nurse on the CT Health Assessment Record, (Blue form).

Physical forms can be downloaded from the Board of Education Website under Health Services. Forms do not have to be submitted on blue paper.

Early Dismissal: Only dismissals for illness and Medical appointments are handled through the Health Office. All other dismissals are handled by your child’s house office.

Requesting Early Dismissal: The best way to assure that your child will be dismissed at the necessary time is to call the health office in the morning and request the dismissal.  The earlier you call for the dismissal the more likely we will dismiss them on time.  If you have called us for the dismissal, you do not need to come in to the school.  You may pull up to the front entrance and your child will come out to meet you.

Medical Notes requiring adjustments to academics or activities: If your child needs to be excused from gym or sports for an extended period of time, you will need to supply a doctor’s note stating the amount of time they will need to be excused AND the date of their reevaluation.
