Registration
Ready to Become a Bear?
Before hitting the field, court, track, pool, or course, all Norwalk High School student-athletes must complete the athletic registration process through FinalForms.
Don't wait until the season starts!
Completing these requirements early can help avoid delays when practices and tryouts begin. Start your registration, check that your physical is current, and make sure all required forms are complete.

Get Started
1. Register in FinalForms
A parent or guardian must begin the registration process in FinalForms and select the sport(s) their student plans to participate in.
Planning to play more than one sport? We strongly recommend selecting all anticipated sports for the school year when you first register. This helps prevent registration issues or delays later in the year.
2. Complete All Required Forms
Registration requires action from both the parent/guardian and student-athlete. Be sure all required forms, signatures, and acknowledgments are completed before participation.
3. Submit a Current Sports Physical
Every student-athlete must have an active sports physical on file with the school nurse. A student may not participate without a valid physical.
The Connecticut State Department of Education Health Assessment Record needs to be completed in order for your child to participate in any school sport at Norwalk High School. The form is a combination of health and sports and must have the area at the bottom of page two completely filled out. See this section below.

4. Meet Academic Eligibility Requirements
Student-athletes must meet the school's minimum academic standards to be eligible to participate in athletics.
